Re:  IPMG Case Management Transition

 

Indiana Professional Management Group, Inc. (IPMG) has recently been selected to be the statewide provider of Case Management services for individuals who participate in the Developmental Disabilities, Autism and Support Services waivers.  It is with much enthusiasm that IPMG desires to partner with you to ensure that the Case Management services you are receiving continue at the quality level desired.

 

IPMG will be contacting you individually to let you know if your current Case Manager will be available to provide your Case Management Services.  If you would like for your current Case Manager to continue providing your Case Management services, you need not take any action.  Your Case Manager will assist you with any required signatures to confirm your choice.

 

If your current Case Manager will not be available, or if for any reason you would like to interview other choices for Case Management services, your IPMG district Case Management Organization (CMO) will provide you with a Choice Statement.  The Choice Statement will list the names of Case Managers who are located near you and who possess the expertise to meet your needs. An IPMG Supervisor will be available to assist you as needed in the interview process. 

 

IPMG is committed to choice and consumer satisfaction.  IPMG wants to ensure that your services are consumer driven at all levels.  Your Case Management services will continue to be delivered during this process to ensure that your health, safety and other needs are met at all times.
