Indiana’s Division of Disability and Rehabilitation Services (DDRS) has announced that effective January 1, 2022 a total of six case management agencies will be authorized to provide case management services to people receiving Medicaid Waiver services through the Family Supports Waiver and Community Integration and Habilitation Waiver.

The following agencies have been selected to provide case management services. Individuals receiving case management services through an agency that was not selected will need to select a new case management agency by December 14, 2021.
