Medicaid Waiver Case Management Organization Changes

4.28.26 Update – Preliminary contracts have been awarded to case management organizations (CMOs) approved to provide case management services to people on the Community Integration and Habilitation (CIH), Family Supports (FSW), Health & Wellness (H&W), and Traumatic Brain Injury (TBI) Medicaid Waivers. This does not affect the PathWays for Aging Waiver.

The following are the approved CMOs:

  • Aging & In-Home Services of Northeast Indiana (AIHS )
      Note: AIHS WILL provide services statewide
  • Indiana Professional Management Group (IPMG)
  • Inspire Case Management
  • The Columbus Organization
  • Unity of Indiana

If your CMO is not on this list, it means that the state did not select them and will not continue providing case management services.

The following is information about this change and information on how to choose a CMO.

What do I do if my current CMO was not selected?

If you currently receive case management services from a CMO that will not continue, you must select a new CMO by July 15, 2026. If you do not select a new CMO, you will be auto-assigned to one of the approved organizations.

It is important to know that you can always change your CMO at any time, for any reason.

If my CMO was not selected, can I follow my current case manager to a new organization?

If you like your case manager and the CMO they work for was not selected, talk with them. Ask if they are moving to another organization and the name of the organization. You can choose that new organization if you wish. Remember, this is an opportunity to look at other CMOs. You can always switch CMOs or case managers at any time for any reason.

My CMO was selected. Can I keep my case manager?

Yes. If your CMO was selected, you should be able to keep your case manager. 

My CMO was selected, but I don’t like my case manager. Can I change case managers?

Yes. You always have the right to change your individual case manager or CMO.

Will I lose my waiver or any of my other services?

No, you will not lose your waiver or any of your other services because of the changes in approved case management organizations.

Choosing a Case Management Organization

The process for selecting a new CMO for individuals whose current organization is not continuing has not yet been announced. The state plans to send a toolkit next month to those affected and will host “meet and greet” events across the state with the approved CMOs.

Currently, you can request a “pick list” of approved CMOs by contacting your local Bureau of Disability Services (BDS) office or your current CMO. After reviewing your options, you then return the completed pick list to BDS or your current CMO with your selection indicated.

When choosing a Case Management Organization (CMO), it’s important to take time to research the organizations that provide case management authorized through the Medicaid Waiver program. This will help you decide which CMO will be the best fit for you or your loved one  You can call or e-mail a CMO to ask questions to learn more. While you may not be speaking to the person who would be your case manager if you select that organization, you can learn about the organization as a whole.

Before you begin, if you are researching CMOs on behalf of another person, ask them what is important to them and what questions they would like you to ask. If possible, include them in the conversation.

The following are suggestions to consider.

Review the organization’s website for information about its mission and its vision for providing case management services.

If possible, connect with others who receive or have received services from the CMO. This might include people you know from Special Olympics, Self-Advocates of Indiana, local chapters of The Arc, parent groups, etc.

If you reach out to the organization, are they open to talking to you?

If you leave a voicemail, how quickly do they return the call? Please be aware that because so many people will be affected by this change, it may take longer than usual to receive a call back.

The following are suggested questions to ask or look for in your research.

  • What can you do for me or my loved one?
  • What sets your company apart?
  • What qualifications or training do your case managers have?
  • What ongoing training do they receive?
  • Will the case manager use my preferred method of communication (text message, e-mail, phone call)?
  • How often will you communicate with my family and me?
  • What is the process for addressing concerns or complaints?
  • Are your case managers experienced in supporting individuals with my or my loved one’s specific disability, including co-occurring conditions like developmental disabilities, TBIs, physical disabilities, and mental or behavioral health needs?
  • How will you help me understand and connect to formal and informal services and community resources that meet my or my loved one’s needs?
  • How will your case managers support me or my loved one to lead my team meetings and make decisions that support my or my loved one’s vision of a good life?
  • How do your case managers help coordinate services from different providers to make sure everyone is on the same page?
  • How do you ensure self-advocates have the support they need and want in team meetings and in managing their waiver services?
  • How do you include family members of adults?
  • How do you manage caseloads to ensure my case manager can respond promptly and stay accessible when I need support?
  • How will you help me or my loved one navigate the changes occurring with Waivers and Medicaid?
  • How will you help me or my loved one if there is a problem with Medicaid or Social Security?
  • What is the process for reaching my case manager after hours or on weekends if there is an urgent issue?
  • If my case manager leaves, how and when will I be notified, and what is the transition plan?
  • What is your staff turnover rate? 
  • If I want to change my case manager, what is your process to do that?
